Please fill out attached registration form and return it to the Biddeford Recreation Department. The form can be mailed directly to the recreation office at:

Biddeford, ME 04005
or scanned and emailed to cubcare@biddefordmaine.org. If you would like to drop off the form at the recreation department please call 207-283-0841 to make sure a staff member will be available to receive the form. Once received a member of the recreation department will be in touch via phone call or email to let you know that you are all set to continue the process.
